By checking this checkbox, I confirm that I understand and agree to the following:

 

Age Confirmation

 

I am at least 18 years old and will provide a valid government-issued photo ID before receiving a tattoo. 

 

I am between 12 and 16 years old and I have permission from my parents to get this tattoo.

 

 Health Disclosure

 

I am not under the influence of alcohol, drugs, or blood-thinning medications.

 

I am not pregnant or nursing.

 

I have disclosed any medical conditions (e.g. skin issues, allergies, diabetes, epilepsy, hemophilia, etc.) that may affect the tattoo process or healing.

 

I understand it is my responsibility to consult a physician if I have health concerns.

 

Tattoo Process

 

SI acknowledge that receiving a tattoo involves risks, including infection, allergic reaction, scarring, and possible skin irritation.

 

I understand that final placement and design must be approved by me before tattooing begins.

 

I understand that tattoos are permanent and that the artist will perform the procedure to the best of their ability, but healed results can vary depending on skin type, placement, and aftercare.

 

 Aftercare

 

 I have received and understand the aftercare instructions provided.

 

I agree that it is my responsibility to properly care for my tattoo and that INK ON PART is not liable for healing issues due to improper care.

 

 Touch-Ups

 

 Touch-ups are offered at the artist’s discretion and may incur a fee, especially for tattoos on fingers, hands, feet, or ribs.

 

Touch-ups must be scheduled within 5 weeks of the original appointment.

 

 

Payment & Refunds

 

 I understand that all deposits are non-refundable.

 

Cancellations or no-shows with less than 48 hours’ notice will result in loss of deposit.

 

I understand there are no refunds on tattoo services.

 

 Media Consent

 

 I give permission for photographs and videos of my tattoo to be taken and used for portfolio, website, and social mediapromotion.

 

Liability Waiver

 

 I release INK ON PART, the tattoo artist, and studio staff from any and all claims, damages, or legal actions arising from this procedure.

 

 Client Acknowledgment

 

I have read and understood the above terms. All my questions have been answered to my satisfaction. I agree to proceed voluntarily.
